At Hometown Connect, we understand the challenges of addiction and the importance of a comprehensive approach to recovery. With a focus on holistic healing, we provide comprehensive services that address the physical, emotional, and spiritual aspects of addiction. We are passionate about going the extra mile to help break the cycle of addiction we see in so many of our local communities. We work with recovery across the state of Kentucky and beyond and are heavily involved in the discharge planning and re-entry. We believe in empowering individuals to overcome addiction and rebuild their lives.
Reclaim Your Life Through Addiction Recovery
Hometown Connect addresses social determinant barriers and assesses patient needs so that they can connect them to established resources. We visit recovery centers regularly to provide Connect Re-entry Classes, Bibles and Blessing Bags. Our hope is that when a patient leaves a facility and is partnered with a mentor to help them take their next steps, clinically and socially, it will break the cycle of relapse or re-entry and allow the patient to thrive and grown on their own.
